The post-holder will be part of a multi-disciplinary team (Social Workers, Occupational Therapists, and Physiotherapists) which provides a health and social care service to the local community. In general Case managers will: • be expected to care coordinate a caseload of people with varied complexity of needs The post-holder plays a lead role in coordinating the care and support for users (and their carers/ families) pulling in the expertise of other professionals as required They will also be responsible for accurate and comprehensive inputting of case management information for monitoring and planning purposes The post-holder will aim to provide an efficient and cost effective health and social care service to service users, carers and their families, in accordance with agreed threshold criteria and using appropriate resources of Social Services, Health, the Council, the community and other non-statutory organisations. £22.98 per hour.
