Occupational Therapist – Adult Intake Team Location: Barking & Dagenham, UK
Salary: £32–£35 per hour
Hours: 35 hours per week
Contract Type: Full Time, Ongoing
Working Pattern: Hybrid – 3 days in the office, 2 days WFH
Start Date: ASAP

Barking & Dagenham’s Adult Intake Team is seeking a qualified and experienced Occupational Therapist to support the delivery of timely, person-centred assessments and interventions for adults with care and support needs. This role is ideal for a confident practitioner with strong assessment skills and experience within adult social care or community settings.
You will work as part of a fast-paced intake service, completing initial assessments, identifying risks, and supporting individuals to maintain independence through appropriate equipment, adaptations, and care planning.
Key Responsibilities:
  • Complete strengths-based initial assessments for adults referred into the Intake Team.
  • Identify functional needs, risks, and appropriate interventions to promote independence and wellbeing.
  • Recommend and arrange equipment, minor adaptations, and signposting as required.
  • Work in partnership with social workers, community teams, and external agencies to provide coordinated support.
  • Maintain accurate and timely records in line with statutory requirements and professional standards.
  • Contribute to safe and effective discharge planning, urgent referrals, and immediate risk management when required.
Essential Requirements:
  • Qualified Occupational Therapist (Diploma/BSc/MSc).
  • Post-qualifying experience within adult social care, community OT, reablement, or related settings.
  • HCPC registration.
  • Confident undertaking functional assessments, risk management, and equipment recommendations.
  • Strong communication, judgement, and caseload management skills.
  • Ability to work onsite 3 days per week and 2 days from home.
