Reablement Enabler, Islington 
The post holder will deliver personal care and support to people in accordance with their personalised support/Reablement plans. This may include personal care tasks, domestic tasks or exercise programs, developed by therapists to support people to achieve agreed outcomes and add to their rehabilitation or recovery at home. Care and support are given in an enabling way and the focus is on enablement and the promotion of independence. The Enabler will work closely with health and social care professionals, who are involved in the care planning and delivery of Reablement care. The post holder will report directly to the Reablement Senior Enabler/Coordinator. Previous experience in a similar role is essential.  
Pay - £22.98ph
